Patient Medical Homes and Primary Care Networks

As our province moves towards an integrated system of care via the patient medical home (PMH) and primary care network (PCN) models, there will be great value in divisions supporting each other through collaboration and resource sharing.

The General Practice Services Committee (GPSC) has created several tools to help divisions communicate about patient medical homes (PMH) and primary care networks (PCN).

Collaborative and shared resources

The Community Practice, Quality, and Integration (CPQI) department used an appreciative inquiry approach to document communities’ progress towards the new model, asking divisions and their partners if they had PMH/PCN resources to share on both a community and practice level. We discovered that great work is already underway in many communities and there have been a variety of resources developed and deployed.
