Partnering with Abbotsford’s Fraser Health leadership, Abbotsford Community Services and community stakeholders, the Division is engaging its Members in co-designing improved pathways and clinical service delivery models that provide patient centered care. Areas of work include team based care, networks and conduits to specialized services. Work in the last two years has focused on the frail senior population and will now shift to include mental health access.
