Long Term Care Initiative (LTC)

LTC enables community, family-physician groups to improve patient residential care services with local solutions.

Lead physicians collaborate with Interior Health (IH) and other community partners. They work on best practices and addressing system-level issues. The steering committee meets quarterly with IH and Golden Life (senior living centres) staff to improve the quality of patient care.

 

Project Profile

Local teams ensure long-term, care facility patients have medical coverage. Their goals include:

  • 24/7 availability and on-site attendance when required
  • Proactive visits to residents
  • Meaningful medication reviews
  • Completed documentation
  • Attendance at case conferences
  • Reduced unnecessary or inappropriate hospital transfers
  • Improved patient-provider experience
  • Reduced cost/patient as a result of a higher quality of care
   

Community Approach

Each community chooses its own approach. While the focus is on patient care, success depends on building strong relationships and shared goals with local, health care partners.

All EKDoFP communities have LTC projects: Creston, Cranbrook, Kimberley, Golden, Invermere, Fernie, Elkford and Sparwood.
