The Program Director is responsible for overseeing and managing a specific set of programs. The primary role is to ensure the successful planning, implementation, and evaluation of programs to achieve the Division and the funders goals and objectives. This role provides oversight on all Division Shared Care projects, Patient Medical Home supports and services and the organization's evaluation and reporting of all programs and projects. Langley Division: Attachment Coordinator Open until filled

The Attachment Coordinator is responsible for supporting BC residents who have registered on the Health Connect Registry (HCR) (i.e., unattached patients) to connect with a primary care provider (i.e., FP or NP) within their community of residence who can provide the resident with longitudinal primary health care services. The Attachment Coordinator is responsible for proactively engaging and communicating with community members on attachment status and processes, as directed by policy and program guidelines. This position will be required to travel to clinics to support EMR record entries.
